This appointment is for weight loss clinic start-up. If unsure which program best fits your needs, this should be preceded by "Weight Loss Clinic Consultation". At this appointment, we will complete any exams and/or labs based on program chosen during consultation. We will also order medications if applicable and discuss diet and exercise plan. Follow-up appointments are also scheduled. Full plan payment is due at this appointment. Financing is available through Cherry if approved. Non-refundable deposit invoice of $50 will be sent prior to appointment to hold appointment. All appointments cancelled within 8 hours of appt are subject to $25 reschedule fee. Please do not arrive early for treatment or bring children or guests.
